The Exclusive Plus plan is the best plan we offer with comprehensive health insurance for foreigners in the Czech Republic that exceeds the requirements necessary to obtain a visa or residency permit.
This plan is administrated in the Czech Republic for necessary healthcare locally, the EU-Schengen Coverage Area and optionally Worldwide. Private clinics are administered by AccessHMO.
Please read the Terms & Conditions carefully before selecting your plan and kindly refer to this guide in the future when in need of medical care at Hospitals, Clinics and Pharmacies.
Pre-existing medical conditions are Not Covered. However, your application will be approved for all other covered expenses.

Maximum benefit payable is 10,000,000 CZK. per policy period outside of the Czech Republic and
€60,000. per each medical or accident event within the Czech Republic Limited to Emergency Only Coverage in the EU-Schengen Zone.This plan provides both outstanding access and coverage in the Czech Republic as well as in countries beyond the EU-Schengen Zone including the U.S., Canada and UK
at superior medical facilities with same-day medical access.
IMPORTANT: Your individual coverage area are the destinations you entered in the “Travel Itinerary” box on your application form where requested. Please notify us at claims@hamiltonhudson.cz should your travel plans change, especially if you are planning to visit the U.S.
Approved medical facilities accept our Exclusive Plus Plan after you have phoned the AXA Assistance telephone number on the back of your card for verification of your coverage and to pre-certify your medical need and schedule your doctors appointment at the nearest participating medical facility.

To schedule an appointment at private clinics not listed by AXA Assistance, please contact Poliklinika Na Narodni, Child Friendly or Canadian Medical directly and forward your appointment details to
claims@hamiltonhudson.cz to arrange a Direct Payment on your behalf.
All applications must be submitted with full premium payment for the entire coverage period.
The Czech Republic forbids installment or monthly billing and there are no refunds for premature policy cancellations or changes in plan selection.
